What to expect at your New Patient Evaluation
The initial appointment with our providers is a comprehensive diagnostic evaluation.
Patients are scheduled for a one hour in office appointment.
Prior to the appointment, patients need to complete a set of online forms that cover medical and behavioral questionnaires. These forms cover patient and family medical and behavioral history.
Patients are providing a medical diagnosis and treatment choices are discussed.
Treatment plans could include medication or therapy or both.
A few treatment plans may include a recommendation for a higher level of care for patients where outpatient office visits are not sufficient.
Follow up visits are scheduled based on the treatment plans.

What to expect at your Follow up Visit
Based on the treatment plan, follow up visits are scheduled to review progress and medication management.
Follow up visits can be a Telehealth or In office visit.
These appointments are 20-30 minutes.
Patients are required to be in these appointments.
Children & Adolescents are required to attend along with a guardian.
Patients are required to login to their Patient portal before the follow up visit to complete any forms assigned.
