What to expect at your New Patient Evaluation

  • The initial appointment with our providers is a comprehensive diagnostic evaluation.

  • Patients are scheduled for a one hour in office appointment.

  • Prior to the appointment, patients need to complete a set of online forms that cover medical and behavioral questionnaires. These forms cover patient and family medical and behavioral history.

  • Patients are providing a medical diagnosis and treatment choices are discussed.

  • Treatment plans could include medication or therapy or both.

  • A few treatment plans may include a recommendation for a higher level of care for patients where outpatient office visits are not sufficient.

  • Follow up visits are scheduled based on the treatment plans.

What to expect at your Follow up Visit

  • Based on the treatment plan, follow up visits are scheduled to review progress and medication management.

  • Follow up visits can be a Telehealth or In office visit.

  • These appointments are 20-30 minutes.

  • Patients are required to be in these appointments.

  • Children & Adolescents are required to attend along with a guardian.

  • Patients are required to login to their Patient portal before the follow up visit to complete any forms assigned.

  • Self-pay

    • New patient evaluation: $400

    • Follow up visit: $250

Billing: Per our office policies, all patients are required to store a credit card on file.

  • Self-pay: Payment is expected before the appointment.

  • Insurance: Our Billing depart will submit claims to your insurance, and after the insurance processes the claim, we will charge the Patient responsibility to your credit card on file.

  • Note: Depending on the claim processing time, there will be a lag in our charging your card on file for your patient responsibility.

  • Patient statements are provided upon request.

  • Cancellation and No Show Fees will be charged per our office policies.
