Existing patients: Patient Portal (MYIO)

MYIO helps you stay connected to your providers and staff to:

  • Complete Assigned measures and forms

  • Communicate with your provider by sending a provider message, including sending attachments.

  • Check your patient statement, make payments, or change a payment method

  • Update insurance details

  • View your appointments

  • Request medication refills

New Patients: Request an Appointment

  • New patient requests can be made by filling the form listed on this page. Online requests take precedence over direct phone call requests.

  • All new patient requests go through an intake assessment, our staff will call patient families and explain the process, check if we provide the services you are looking for, insurance coverage/costs, scheduling availability, etc. This call usually lasts above 15-20 minutes.

  • We do not provide counseling or therapy services, we provide psychiatric evaluations and medication management.

  • A patient portal account needs to be set up for all patients, including new patients.

  • A set of online forms made available through the patient portal need to be completed before the new patient visit
